The client participates
in establishing his/her
personal care plan.
Personal care involves
all aspects of personal
hygiene and may include
all or part of the
following:
Back Massage Bathing Dressing
Foot Care Hair Care
Peri-Care
Nail Care Oral Hygiene Shaving
Meals will be prepared
and served by the
caregiver at the
client's request. Meals may also be
prepared in advance for
the client's
convenience.

locally as well as to
Manchester and Nashua. I

Autumn Passages will provide
general housekeeping for
clients receiving specific
services of four (4) or more hours on a regular basis.
The client will receive
personal attention from a
trained and experienced
caregiver.
Staff
are
flexible and participate in
requested activities as
determined by the client.

Due to the increased demand
for transportation to
medical appointments,
shopping, luncheons, church, and
errands by clients, Autumn
Passages has a company van
with an electric lift seat to
assist with entry and exit
for those needing
assistance.
The van is scheduled as
client needs dictate and is
driven by designated staff
with approved driving records.
Wheel chairs, walkers and
canes are transported along
with our clients to their
destination. Outings
throughout the Monadnock
Region have become a regular
part of clients daily
activities.
Clients personal vehicles
may also be driven for their
own comfort by designated
staff.
Available
to clients needing assistance with Activities of
Daily Living of up to one hour, ie., supervision of
medication, a.m. or p.m. care, meal preparation in
addition to an "overall safety check in".

to participate with the
caregiver on shopping trips
for exercise and
socialization. Shopping
services are offered to
those clients requiring
regular assistance in
obtaining:
Banking
Clothing
Groceries
Household Items
Medical Supplies
Medications
Personal Items
Pet Supplies
Postal Supplies
Autumn Passages specializes
in providing personal and
individual care for those
people requiring supportive
services twenty-four hours
per day. Complete personal
care plans and home
maintenance tasks are included
in the services offered by
Autumn Passages Eldercare.

Physical, Occupational and Speech Therapies are offered
as physician orders dictate through
referrals.
Range of Motion is provided to clients after their
therapist discharges them. This daily exercise
maintains the clients mobility.
Swimming therapy
is available upon request. Local lodging
establishments have heated pools/jacuzzi where memberships
are available. Dartmouth Hitchcock Hospital in Keene
has therapy pools where aquatic programs are
offered.
At the request of the
client/family, Autumn
Passages will provide
personal care in addition to
all other services offered
in all facilities throughout
the Monadnock Region to
include Senior independent
apartment complexes.
In addition to the extensive
list of services, Nancy
offers her clients extra
personal attention. Clients
have enjoyed theme based
luncheons, holiday parties
and the ever favorite....
Kimball's Ice Cream outing
(in season) at Silver Ranch
in nearby Jaffrey.
