Who we are
An independent team with no financial relationships with home health agencies. We accept no listing fees, no sponsored placements, and no payments of any kind from the agencies in our directory. Our only job is to help families make informed decisions.
Our data source
Every agency is sourced from CMS Home Health Compare — the same federal database used by Medicare itself. We update our data quarterly to reflect the latest quality scores and HHCAHPS patient survey results across 9,355+ certified agencies nationwide.
Our promise
Autumn Passages is free for families, always. We don't sell your information, we don't require an account to search, and we never share your contact details beyond the agency you specifically request a callback from. Privacy-first, full stop.
Our Data Sources
All agency data on Autumn Passages is sourced from the CMS Provider Data Catalog, the official federal repository maintained by the Centers for Medicare & Medicaid Services. This is the same data Medicare uses internally and publishes publicly for consumer transparency.
Agency quality scores are drawn from two primary CMS datasets:
- Home Health Agency Quality Measures (dataset ID: 6jpm-sxkc) — clinical outcome measures and process-of-care metrics reported at the agency level.
- Home Health Care National Averages (dataset ID: 97z8-de96) — national benchmarks used to contextualize each agency's performance relative to the broader industry.
- HHCAHPS Patient Survey Results — the Home Health Consumer Assessment of Healthcare Providers and Systems survey, which captures patient experience directly. Survey star ratings reflect how patients rate communication, professional care, and overall experience.
CMS updates this data quarterly. We refresh our database each time CMS publishes a new release to ensure the quality scores, star ratings, and agency details you see are current.
How We Evaluate Agencies
Every ranking and star rating displayed on Autumn Passages comes directly from CMS-published data. We do not create our own proprietary scores, and no agency can pay to improve its ranking or placement.
The Quality of Patient Care star rating is assigned by CMS and is based on a composite of outcome and process measures across several domains, including:
- Patient functional improvement in walking and mobility
- Improvement in bathing and self-care activities
- Timely medication management and medication teaching
- Hospitalization rates and discharge-to-community outcomes
- Preventable readmission rates (claims-based measures)
These measures are calculated from Medicare claims data and OASIS assessments (Outcome and Assessment Information Set) submitted by agencies to CMS. The methodology is described fully in the CMS Home Health Quality Initiatives documentation.
In addition to quality star ratings, we display HHCAHPS patient survey scores, which reflect the direct experiences of patients who received care. These include patient-reported ratings of professional care, communication quality, medication discussions, and willingness to recommend the agency. Survey participation is conducted by approved CMS vendors and results are publicly reported.
